Sending a Case

Thank you for choosing CDT for your restorative needs. We appreciate your taking the time to fill out the script completely, marking all appropriate information. Finished cases will be delivered to you on or prior to the day requested on the lab script. Please remember to factor in transit time when scheduling patient.

Pick-Up and Delivery

If your office is in the greater Cleveland / Lorain area, CDT will be happy to pick-up your cases. We can set up your office as a convenient daily stop, or on an as-need basis. All you need to do is call us at: 440-324-7704.

Shipping a Case

For offices outside our route area, we offer pick-up and delivery services through UPS Ground, FedEx and the US Postal Service. Please consider sending multiple cases at once to save on shipping.

CDT will pay to ship cases to you. Our clients have the responsibility to pay for shipping cases to the lab. Please remember to count days in transit when scheduling patient.

Please... place rinsed, blood- and saliva-free, properly disinfected impressions in sealed plastic bags, separate from script and models.

In-Lab Days Do Not Include Pick-Up and Delivery Days, Weekends or Holidays. Please add additional time if scheduling out of town.
